Toad medicine is the colloquial name for 5-MeO-DMT, a potent short-acting serotonergic psychedelic. Once discussed almost exclusively in ceremonial contexts, it is now the subject of a growing body of academic research examining its effects on depression, anxiety, and subjective wellbeing. This article summarizes what the current evidence actually shows, what it doesn't, and how Sanctuary Tulum incorporates 5-MeO-DMT into a physician-supervised protocol.

The Molecule and Its Traditional Context

5-MeO-DMT (5-methoxy-N,N-dimethyltryptamine) is a naturally occurring tryptamine found in trace amounts in several plants and in the parotid gland secretions of the Sonoran Desert toad (Incilius alvarius, historically Bufo alvarius). It also occurs endogenously in mammals and can be manufactured synthetically. When vaporized or insufflated, the experience is unusually short — typically 15–45 minutes — and unusually intense, characterized by rapid onset, ego dissolution, and a peak that many participants describe as among the most meaningful experiences of their lives.

What the Research Shows

A 2019 survey study led by researchers at Johns Hopkins, published in American Journal of Drug and Alcohol Abuse, examined 362 adults with a history of 5-MeO-DMT use. A majority reported improvements in depression and anxiety symptoms following their experience, and rated it among the most personally meaningful and spiritually significant events of their lives1. A subsequent observational study in Psychopharmacology reported that a single inhalation of vaporized 5-MeO-DMT in a group retreat context was associated with sustained reductions in depression, anxiety, and stress and increases in mindfulness and life satisfaction2. A review of preclinical and clinical work on 5-MeO-DMT was published in Frontiers in Neuroscience in 20223.

These findings are meaningful and consistent, but they are not the same as evidence from large randomized controlled trials, which are still ongoing. Sanctuary Tulum describes 5-MeO-DMT as a promising, physician-supervised experiential intervention supported by early evidence — not as an approved treatment for any specific psychiatric diagnosis.

How 5-MeO-DMT Acts on the Brain

5-MeO-DMT is a potent agonist at serotonin 5-HT1A and 5-HT2A receptors — the same receptor family targeted by psilocybin and LSD, but with markedly different pharmacokinetics and subjective effects. Neuroimaging research on classical serotonergic psychedelics has consistently documented acute reductions in default mode network activity, and this is one leading hypothesis for the ego-dissolution phenomenology 5-MeO-DMT produces. Larger direct-imaging studies of 5-MeO-DMT specifically are still in progress.

Sourcing and Conservation

Wild-toad sourcing raises serious conservation and welfare concerns. Overharvesting has been reported to threaten Incilius alvarius populations in parts of the Sonoran Desert, and unregulated collection introduces additional variability and contamination risk. Synthetically produced 5-MeO-DMT is pharmacologically the same molecule and does not depend on wild toad populations. This is the material used inside Sanctuary Tulum's ceremonies.

Safety and Contraindications

The physiological safety profile of 5-MeO-DMT is favorable when careful screening and supervision are in place, but the intensity of the experience creates real psychological risk if it is not properly facilitated. Reports of adverse events cluster in unsupervised or under-supervised settings. Absolute contraindications include current SSRI or MAOI use, lithium use, uncontrolled cardiovascular disease, and personal or family history of psychotic disorders. Every guest at Sanctuary Tulum is screened by our clinical team before a 5-MeO-DMT ceremony is scheduled, and every ceremony is physician-supervised with medical staff on-site and integration support built into the days that follow.

Where It Fits in the Pouyan Method™

Within the Pouyan Method™ at Sanctuary Tulum, 5-MeO-DMT is carefully sequenced alongside other modalities. It typically follows earlier ibogaine treatment and ayahuasca ceremonies, and is paired with integration work, IV nutrient support, and structured aftercare.
