Hyperbaric Oxygen Therapy (HBOT) involves breathing 100% pure oxygen in a pressurized chamber at 1.5 to 3 times normal atmospheric pressure. How a Hyperbaric Session Actually Works
A typical session begins with a pressure check and a review of ears, sinuses, and any recent changes in medication. Guests then enter the chamber and breathe either ambient pressurized air enriched with supplemental oxygen or, in a monoplace chamber, 100% oxygen directly, while pressure is gradually increased to the prescribed depth-equivalent — usually between 1.5 and 2.4 atmospheres absolute. The "descent" can produce a sensation similar to the ear pressure changes felt during airplane takeoff, which is why guests are coached on equalization technique before their first session. Once at target pressure, the session itself is calm: most guests read, rest, or simply sleep for the 60 to 90 minute duration.
Decompression at the end of the session is gradual, mirroring the ascent profile used in diving medicine, to avoid barotrauma. A physician or trained hyperbaric technician monitors every session, and vital signs are checked before and after. Because HBOT is delivered daily during a treatment stay, our medical team tracks cumulative exposure across the length of the program and adjusts pressure or duration based on how a guest is responding — the same individualized approach used across the rest of the Pouyan Method™.
What the Research Shows
Preclinical work in stroke and hypoxic brain injury models has repeatedly linked hyperbaric oxygen exposure to measurable increases in neurogenesis. A widely cited rodent study found that hyperbaric oxygen therapy promoted neurogenesis following cerebral injury (Pubmed), and follow-up work on middle cerebral artery occlusion models showed that delayed HBOT still promoted neurogenesis through hypoxia-inducible pathways (Pubmed). A separate study on carbon monoxide poisoning found that hyperbaric oxygen preserved BDNF-dependent hippocampal neurogenesis and reduced delayed memory impairment (Pubmed).
In humans, a randomized controlled trial of HBOT for persistent post-concussion syndrome after mild traumatic brain injury reported measurable improvement in symptoms and cognitive testing among participants who received the active protocol (Pubmed), and a meta-analysis pooling multiple prospective studies of HBOT in traumatic brain injury found a consistent benefit across mild to severe cases (Pubmed). A companion trial specifically measuring cognitive function in TBI patients exposed to HBOT versus sham conditions found changes in both cognition and PTSD symptom scores (Pubmed).
Closer to addiction medicine specifically, a randomized feasibility study tested hyperbaric oxygen as an adjunct for adults reducing their methadone dose during opioid use disorder treatment, and reported that the approach was both acceptable and feasible for this population (Pubmed). These findings are early and hedge accordingly — HBOT is designed to support recovery, brain repair, and cognitive function alongside the rest of a comprehensive protocol; it is not presented as a stand-alone cure for addiction or a guaranteed outcome for any individual guest.
Safety, Screening, and Who HBOT Is Right For
HBOT is generally well tolerated, but it is not appropriate for every guest. Untreated pneumothorax is an absolute contraindication, and conditions such as severe COPD, certain ear or sinus pathologies, uncontrolled seizure disorders, and some implanted devices require careful medical review before a guest begins sessions. This is why every guest at Sanctuary Tulum completes a full medical intake — including relevant imaging and history — before HBOT is added to a treatment plan, and why sessions are physician-supervised rather than self-directed.
