In 2021, Imperial College London conducted a randomized, double-blind trial directly comparing two doses of psilocybin (with psychological support) to six weeks of daily escitalopram, a standard SSRI (Carhart-Harris R, et al. N Engl J Med. 2021). On the trial's pre-specified primary endpoint — change in depression score at six weeks — psilocybin was not shown to be statistically superior to escitalopram; the two groups improved by a similar amount. On several secondary measures — including well-being, social functioning, and anhedonia (the ability to feel pleasure) — the psilocybin group showed larger improvements, but secondary findings are hypothesis-generating, not confirmatory. Psilocybin delivered this result across two dosing sessions, versus daily dosing for the SSRI arm, which is itself a meaningful practical difference even without a primary-endpoint win.

Psilocybin's side effects are limited to the acute experience (nausea, temporary anxiety) and typically resolve within hours. There is no daily dosing and no withdrawal syndrome.
Treatment Duration
Most SSRI protocols involve months to years of daily medication, with many patients remaining on antidepressants indefinitely. Psilocybin therapy typically involves 1-3 sessions with lasting effects. The Serotonergic Interaction Risk: Why SSRIs and Psilocybin Are Not Simply Interchangeable
Psilocybin and SSRIs both act on the serotonin system, and combining them is not a simple substitution to be made without medical guidance. A 2025 scoping review of concomitant antidepressant and classic-psychedelic use found that SSRIs and SNRIs can blunt the subjective intensity of a psilocybin session, while other patients on the same medications have reported unpredictable or amplified responses — the interaction is not uniform and is not yet fully mapped (Tap SC. Every prospective guest at Sanctuary Tulum completes a confidential clinical intake and full medical screening before any protocol is scheduled.
