The Palhano-Fontes RCT and What It Actually Found
The 2019 randomized placebo-controlled trial by Palhano-Fontes and colleagues studied a single ayahuasca session in 29 patients with treatment-resistant depression. Hamilton depression scores dropped significantly by day 1 and remained significantly lower at day 7 versus placebo. The effect size at day 7 was larger than what a standard 6-week SSRI course typically produces. Response and remission rates at day 7 were 64 percent and 36 percent respectively in the ayahuasca arm.
The trial is small — 29 patients — and the follow-up window is short by pharmaceutical standards. But it is a placebo-controlled human trial with a signal strong enough that mainstream psychiatry has taken it seriously. It is the reason serious clinical interest exists in ayahuasca-assisted therapy for depression at all.
How the Model Actually Works Clinically
Ayahuasca is not a self-administered antidepressant. Ceremony sits inside a multi-week clinical arc: medical clearance and medication taper up front, preparation and intention-setting in the days before, physician and facilitator presence during the session itself, and structured integration hours in the days and weeks after. The published outcomes come from that full arc, not from the ceremony alone.
