Alcohol is one of the few substances whose withdrawal syndrome carries real mortality risk. This article describes the clinical timeline that unfolds after cessation in a physiologically dependent individual — the hours of onset, the peak-risk window, the acute recovery period, and the longer subacute course known as post-acute withdrawal syndrome. It is educational content, not a self-detox guide. Anyone with a history of daily heavy drinking, prior withdrawal seizures, or delirium tremens should be evaluated by a physician before stopping alcohol.
Why Alcohol Withdrawal Is Medically Distinct
Chronic alcohol exposure produces adaptive changes in the GABA and glutamate systems of the central nervous system. When alcohol is abruptly removed, GABAergic inhibition falls and glutamatergic excitation rises, producing the characteristic hyperadrenergic and neuroexcitatory syndrome. National Institute on Alcohol Abuse and Alcoholism guidance describes alcohol withdrawal as a condition that requires medical evaluation because severity is not always predictable from drinking history alone.1
Stage 1 — Onset: 6 to 12 Hours After the Last Drink
In individuals with physiological dependence, the earliest withdrawal signs appear roughly 6 to 12 hours after the final drink. Common early features include:
- Fine tremor of the hands
- Anxiety and psychomotor agitation
- Diaphoresis (sweating)
- Tachycardia and mild hypertension
- Nausea and appetite loss
- Insomnia, vivid dreams, and sleep fragmentation
In a supervised setting a clinician begins scoring severity at this stage using the Clinical Institute Withdrawal Assessment for Alcohol, Revised (CIWA-Ar), the validated 10-item scale developed by Sullivan and colleagues in 1989 that remains the standard bedside tool for guiding management.2
Stage 2 — The Peak Risk Window: 24 to 72 Hours
The 24–72 hour window carries the highest risk in the alcohol detox timeline. Two complications drive that risk profile:
Withdrawal Seizures
Generalized tonic-clonic seizures occur in a minority of untreated alcohol withdrawal cases and typically cluster in the first 12–48 hours after cessation. Prior withdrawal seizures are the strongest risk factor for future ones — a phenomenon known clinically as kindling. NIAAA and the American Society of Addiction Medicine describe seizure risk as a primary reason alcohol withdrawal warrants medical monitoring in anyone with dependence.1,3
Delirium Tremens
Delirium tremens (DTs) is the most severe form of alcohol withdrawal — a hyperadrenergic delirium marked by confusion, disorientation, hallucinations, severe autonomic instability, and fever. Onset is typically 48–72 hours after the last drink. Published clinical literature places incidence at roughly 3–5% of alcohol withdrawal cases; untreated DT historically carried substantial mortality, though mortality falls sharply with modern medical management.4 This is the single most important reason self-managed detox is unsafe for anyone with significant dependence.
Stage 3 — Acute Symptom Resolution: Days 3 to 7
For most individuals with uncomplicated withdrawal, acute autonomic and neuroexcitatory symptoms peak in the first 72 hours and steadily de-escalate over days 3 to 7. Tremor, tachycardia, sweating, and anxiety recede. CIWA-Ar scores decline. Sleep architecture remains disrupted but starts to normalize. Appetite gradually returns. In a supervised program this is the transition point from acute detox to the beginning of longer clinical work.
Stage 4 — Post-Acute Withdrawal Syndrome (PAWS)
After the acute detox is complete, a subacute picture often follows — collectively called post-acute withdrawal syndrome. PAWS features include:
- Sleep disturbance and vivid dreams that can persist for weeks
- Mood variability, irritability, and anhedonia
- Cognitive fog, concentration difficulty, and short-term memory lapses
- Cravings that come and go in waves rather than continuously
- Autonomic instability with intermittent tachycardia, sweating, or blood pressure variability
PAWS typically resolves gradually over weeks to months as GABA/glutamate balance rebalances and stress-axis regulation recovers. Why CIWA-Ar Matters in a Supervised Program
CIWA-Ar scores 10 domains including nausea, tremor, sweating, anxiety, agitation, tactile disturbances, auditory disturbances, visual disturbances, headache, and orientation.2 The composite score guides clinical decisions in real time — when to increase monitoring frequency, when to escalate care, when a guest is stable enough to reduce observation intensity. Individual Factors That Change the Timeline
- Drinking history — quantity, frequency, and years of consumption.
- Prior withdrawal history — kindling raises the risk of severe subsequent withdrawal.
- Medical comorbidities — hepatic function, cardiac status, electrolyte balance, and nutritional state all influence severity.
- Concurrent medications — especially benzodiazepines, opioids, or other CNS agents.
- Age — older adults typically experience longer and more severe courses.
The Dual-Dependency Complication
A meaningful subset of individuals presenting for alcohol detox are also physiologically dependent on prescription benzodiazepines. What Medical Supervision Provides
In a supervised setting the alcohol detox timeline is managed with continuous nursing observation, physician oversight, CIWA-Ar–guided assessment, monitoring of vitals and hydration, correction of electrolyte and nutritional deficiencies where indicated, and escalation pathways ready before they are needed. When to Seek Emergency Care
Anyone experiencing seizure activity, confusion or disorientation, hallucinations, severe autonomic changes, high fever, or chest pain during alcohol withdrawal should seek emergency medical care immediately. The alcohol detox timeline runs from onset at 6–12 hours, through the peak-risk window at 24–72 hours, into acute resolution over days 3–7, and then into the longer post-acute course of weeks to months.
