Most Expensive Luxury Rehabs in the World
Not a ranking. A look at what actually drives the price of the most expensive programs, and why cheaper and costlier options genuinely differ.
The most expensive rehabilitation programs in the world can run well into six figures for a stay of several weeks, while comparable-length programs elsewhere cost a fraction of that. The gap is large enough that it is reasonable to ask what, specifically, the additional cost buys — and whether it correlates with anything measurable.
This is not a ranking of specific facilities by price, since price lists change and are rarely published transparently. It is a breakdown of the cost drivers themselves, so a family evaluating any program — expensive or not — can tell what they are actually paying for.

Staff-to-guest ratio is the largest cost driver
Clinical labor is the single largest expense in any high-end program. A facility hosting six to ten guests with a full multidisciplinary team — physicians, nurses, therapists, bodyworkers — has a materially higher staff-to-guest cost than one hosting fifty guests with a similarly sized team. That ratio is what allows daily one-to-one hours instead of group sessions, and it is largely irreducible: you cannot buy more attention per guest without either raising price or lowering census.
Sanctuary Tulum keeps a deliberately small guest count and delivers over thirty-five hours of one-to-one clinical and therapeutic work per week per guest, which is only possible at that staffing intensity.

Length of stay changes the total, not just the daily rate
Headline prices are sometimes quoted as a daily rate and sometimes as a full-program figure, which makes comparisons misleading unless you check which one you're looking at. A facility with a high daily rate but a two-week minimum can cost less in total than one with a moderate rate and a six-week program. Duration itself is also a clinical decision, not just a cost variable — the National Institute on Drug Abuse notes that shorter stays are associated with higher relapse risk when the underlying condition has not stabilized.
Ask any program, expensive or not, for the total cost across its recommended minimum stay, not a per-night figure that undersells the real commitment.
Ask what the number includes before asking what the number is. That single question resolves most of the confusion around price.

What is usually included, and what usually is not
This varies by program, but these are the categories worth confirming in writing before admission.
Diagnostics
Often included; imaging or specialist referrals sometimes billed separately.
Medications
Frequently excluded and billed through a local pharmacy.
Family sessions
Sometimes capped at a set number of hours.
Aftercare
Ranges from a written plan to ongoing check-ins for months.
Specialized therapies
IV protocols, HBOT, or advanced diagnostics may be priced à la carte.
Travel and accompaniment
Rarely included; usually the family's responsibility.

Diagnostics and specialized protocols add real cost
Comprehensive intake — bloodwork, nutritional and hormonal panels, cardiac and neurological screening where indicated — is genuinely costly to run properly, and it is one of the more defensible reasons a program charges more. It only adds value, however, if the results change the protocol. A center that runs extensive diagnostics and then delivers a generic program has priced in a cost without delivering the corresponding benefit.
The same logic applies to advanced therapies. Hyperbaric oxygen, NAD+ IV therapy, and stem cell protocols each carry real supply and staffing costs; they are worth paying for when clinically indicated, not when offered as a blanket upsell.

Why cheap and expensive programs actually differ
It is tempting to assume the price difference between programs is mostly aesthetic — nicer rooms, better food. In practice the more consequential differences are structural: guest census, staff qualifications and ratios, medical oversight depth, and whether the program can adapt mid-stay when something changes. Two facilities can look similar in photographs and differ enormously in what actually happens during the day.
This is also why price alone is not a reliable signal of quality in either direction. A high price with a low staff ratio and generic programming is not buying what it appears to; a moderately priced program with a genuinely small census and deep clinical involvement can outperform a far more expensive one on the metrics that matter.
A reasonable way to evaluate cost
Rather than comparing headline prices, ask each program for its guest census, staff-to-guest ratio, what diagnostics are run at intake, what is included versus billed separately, and what a typical day of contact hours looks like. The Substance Abuse and Mental Health Services Administration recommends this kind of concrete, individualized comparison over general reputation when evaluating any treatment option.
